Pathogenesis of Root Pains: It is an autoantigen reaction:
Migration of nucleus pulposus through its natural barrier of annulus ligament => Body identifies it as a foreign body => triggers autoantigenic reaction ( inflammation ) against the mucopolysaccharide matrix on the disc surface => release of lytic enzymes ( phospholipase A2 and PG-E2 => enhanced macrophage activity => degeneration of disc fragements and inflammation at nerve roots => increased mechanical sensitivity => enhanced painful stimulus.
